Here at Breakthrough Transformation Trust we are passionate about seeing young people succeed where they haven’t been able to in the past.
We warmly welcome each young person as an individual and meet them where they are at.
We provide education, therapeutic support and a nurturing environment for children and young people with communication and interaction needs, social, emotional and mental health needs, complex diagnoses, learning difficulties, trauma based needs and other challenges, providing a bespoke and personalised curriculum and timetable designed around the needs of the individual.
We understand that finding and accessing the right provision can be daunting. Below is some useful information that may help you navigate your way through this process.
We are able to take referrals via 3 routes:
The Local Authority SEND Team and/or
Student’s current on-roll school
Career South West or other career services
It is expected that young people will have an EHCP (Educational Health Care Plan).
Visits to our centres will be possible following the receipt of a consult and/or request directly from one of the above referral routes.
Compulsory Aged Education Learners:
Breakthrough are not a registered DfE education provider, and therefore, compulsory aged education learners (Reception to Year 11), must remain on roll at a registered school setting. Breakthrough will work in partnership with the school and can offer up to 15 hours provision per week, across 3 days. The core offer is therapeutic education focused on a transitional plan to enable the young person to re-engage with a registered school setting, if possible. Should your child not be registered at a school setting, you should in the first instance, contact your Local Authority SEND Team to discuss further.
Post 16 Learners:
Towards the end of Y11 and/or at the time of your child’s EHCP review, considering the next steps for their educational journey, you may be advised to look at different educational settings that could be a potential setting for your child moving forward. Should Breakthrough be one of these recommendations, you are welcome to contact us to discuss this further. We will need your case worker and/or the career service details to proceed.
